Change Order No. 3 for the 1994 Subdivision Improvements, Project
No. 94-907A.
BACKGROUND
This project involves street and drainage improvements in four
different subdivisions. The project is approximately 80 percent
complete.
Change Order No. 3 involves two of the four areas, Forest Avenue
located in Woodland/Jones Addition and the Southwestern Mobile Home
Court located at the extension of Hageman Avenue in Nord/Wallerius
Additions. On Forest Avenue, some'stormpipeline is being added to
replace or enhance the existing and proposed drainage system. In
Nord/Wallerius Additions, some minor items are being added for the
relocation of the existing stormwater detention basin in the
Southwestern Mobile Home Court.
The amount of the change order is $15,017.00 or 3.2 percent of the
original contract amount of $469,688.65. The total combined change
order increase on this project is 11.5 percent, which is within the
allowable 15 percent contingency amount.
RECOMMENDATION
It is recommended that the City Commission approve Change Order No.
3 in the amount of $15,017.00

THIS AGREEMENT, made and entered into this 24th day of July ,
1995, by and between the CITY OF SALINA, KANSAS, Owner, and the above
named Contractor, modifies the existing contract dated September 19.1994,
under the above subject, project and project number in the following
particulars to wit:
1. The specifications and drawings for certain street improvements in
the City of Salina, Kansas, dated August - 1994 , are hereby amended in
the following particulars only; all other terms and conditions remain
unchanged.

Pipe (RCAP) to replace or enhance the existing storm
drainage system on Forest Avenue.
54. Add 131 linear feet of 10" storm pipeline for the discharge
from the relocated stormwater detention basin in
Southwestern Mobile Home Court (Hageman Avenue).
55. Add 1 lump sum for the removal and replacement of the
existing gate val~e on the stormwater detention basin the
Southwestern Mobile Home Court (Hageman Avenue).
56. Add 1 lump sum for the relocation of the intake structure on
the stormwater detention basin in Southwestern Mobile Home
Court (Hageman Avenue).
57. Add 1 lump sum for a concrete drainage flume in the
stormwater detention basin in Southwestern Mobile Home Court
(Hageman Avenue).
2. The undersigned proposes to furnish all tools, appliances,
equipment, materials, and labor required to complete the construction in
a thorough, workmanlike, and satisfactory manner in accordance with the
drawings, specifications, and contract documents, for the following
prices:
