PROPOSAL FOR ENGINEERING SERVICES
CITY OF SALINA, KANSAS
(Bridge Inventory and Appraisal Update)
The consulting engineering firm of White, Hunsley and Associates,
with offices located in Salina, Kansas, hereinafter referred to as the
CONSULTANT has reviewed the City bridge inventory and appraisal informa-
tion. Based on this information the CONSULTANT submits the following pro-
posal to the City of Salina, Kansas.
1. The CITY has requested a proposal for engineering services from
the CONSULTANT for an update of the structural inventory and appraisal of
bridges (structures) located in the CITY.
2. The CITY desires the services provided by the CONSULTANT to be in
accordance with regulations prescribed by the Federal Highway Administration
(FHWA).
3. The services performed by the CONSULTANT will include the inspec-
tion, analysis and coding necessary for updating the form for the number of
bridges that are summarized below.
A. Phase I - Field Inspection
The CONSULTANT will provide the necessary field inspection and/or
inventory of nineteen (19) City bridges.
B. Phase II - Office Analysis (Calculations and Completion of the
Structural, Inventory and Appraisal SI&A Forms
Updating nineteen (19) City bridges.
C. Phase III - Coding and Preparing Maps
Updating the nineteen (19) City bridges.
4. The CONSULTANT will perform the following scope of services neces-
sary to update bridges designated in Paragraph 3.
(a) Make a visual inspection of each bridge in the field and record
observations regarding the condition of the members in notes.
(b) Make a visual inspection of foundation settlement, scour around
piers and abutments, stream characteristics, deflection of the
structure, deck geometry, approach roadway alignment, approach
and bridge rail adequacy, and include these observations in the
inspection notes.
(c) Make a structural analysis to determine the load -carrying capa-
city of each bridge as necessary to update the report.

(d) Report the condition and rating for each bridge on the SIM
form with recommended weight limits for posting each bridge.
(e) Prepare estimates of cost for suggested repairs and replace-
ment of bridges as required.
(f) Complete for submittal the services listed in Paragraph 6.
5. The CONSULTANT's services will be based on a visual inspection of
the bridge as of the date of its field report and will not include special
inspections such as field X-rays, ultrasonic testing, radiographic inspec-
tion, borings, excavations, destructive testing of materials and other special
testing methods. Testing of this nature shall be performed at such locations
only as specifically and mutually agreed between the CONSULTANT and CITY with
the cost of such testing to be paid for by the CITY or by the procedure es-
tablished in Paragraph 13.
6. The CONSULTANT's proposed services include, as a minimum, submittal
of the following:
(a) All data in a report for the CITY.
(b) For the CITY, a complete file of field notes, structural
analyses, and other supporting information on each bridge used
by the CONSULTANT in determining the condition and the weight
limits for the bridges.
(c) For each bridge, two (2) completed Structural, Inventory and
Appraisal (SI&A) forms.
(d) For each bridge, one (1) completed six -card coding form.
7. The CONSULTANT will schedule the performance of services to include
submittal of the necessary forms within sixty (60) calendar days after re-
ceipt of the written Notice to Proceed.
8. The CONSULTANT will prepare revised forms or make corrections as
requested by the CITY made necessary by errors, omissions or negligence
of the CONSULTANT, at any time prior to the granting of final approval by
the CITY.
9. The CONSULTANT will save the CITY harmless from all damages to
persons or property caused by the CONSULTANT, its agents, employees or sub-
contractors which may result from their operation in connection with the
work to be performed hereunder.
10. The CONSULTANT will comply with all Federal, State and local laws
and ordinances applicable to the work to be performed hereunder.
11. The fee proposed by the CONSULTANT for the performance of services
are as follows:
(a) The Lump Sum Amount of $1,425 which includes $75.00 per bridge
for nineteen (19) bridges.

(b) For the same services for bridges other than the quantity
specified above, the Lump Sum Amount shall be adjusted (plus
or minus) the above -noted fee per bridge. An adjusted increase
(plus) in number of bridges must be approved by the CITY.
12. The CONSULTANT will provide engineering services outside those set
forth above, or for changes in criteria, at a mutually agreed lump sum amount
or at mutually agreed hourly rates at the option of the CITY.
13. Payment to the CONSULTANT should be made within thirty (30) days
after receipt of proper billing and final approval by the CITY.

AGREEMENT FOR ENGINEERING SERVICES
THIS AGREEMENT, made this 7'`'y day of1982,
by and between The City of Salina, Kansas, hereinafter called the CITY, and
White, Hunsley and Associates, Consulting Engineers and Planners, hereinafter
called the CONSULTANT:
WHEREAS, The City of Salina, Kansas, intends to perform an update of the
inspection of the bridges in accordance with standards as designated by the
Federal Highway Administration and the Kansas Department of Transportation
and the CONSULTANT agrees to perform the various professional engineering
services as outlined in the scope of services.
It is agreed that the CONSULTANT perform the services as outlined in
the Proposal for Engineering Services.
IN WITNESS WHEREOF, the parties hereto have affixed their hands and
