Change Order No. Five (5) on the 1995 Subdivision Improvements, Project No. 95-938.
BACKGROUND;
The attached change order modifies the contract with Stevens Contractors approved by the City
Commission on October 9, 1995 for subdivision improvements at eight new or existing subdivisions
throughout the city. The change order includes some minor storm drainage improvements on Seitz
Drive near Melrose Lane. These improvements will attempt to resolve some storm drainage system
problems identified in recent years by area property owners. This work is being proposed by change
order because the current contract already includes nearby street and drainage improvements on Seitz
Drive as well as Eastborough Road.
The cost of'this change order is $19,652. The original contract amount for Project No. 95-938 was
$1,264,525. The total of'the other £our change orders is $282,610. I£this change order is approved,
the new contract amount will be $1,566,787. The city at-large share of' this contract, with the
exception of'the new park assessments in GICO Addition is $280,000. The majority of'those costs
are associated with water and sanitary sewer system improvements. All of' the proposed Change
Order No. 5 costs of'$19,652 will be paid from the general ~und.
RECOMMENDATION;
It is recommended that the City Commission approve Change Order No. Five (5) on the 1995
Subdivision Improvements, Project No. 95-93 g, in the amount of' $ l 9,659_.

THIS AGREEMENT, made and entered into this ~ day of June ,
1996, by and between the CITY OF SALINA, KANSAS, Owner, and the above
named Contractor, modifies the existing contract dated October 9.19950
under the above subject, project and project number in the following
particulars to wit:
1. The specifications and drawings for certain street improvements in
the City of Salina, Kansas, dated September 1995 , are hereby amended in
the following particulars only; all other terms and conditions remain
unchanged.
(a) The scope of the Change Order No. 5 involves an improvement of
existing storm drainage system on Seitz Drive directly north of Melrose
Lane. In recent years, heavy storms have exceeded the capacity of the
existing storm drainage system at this location.
(b) This work is being done by change order to this project because the
project already includes nearby street and drainage improvements.
2. The undersigned proposes to furnish all tools, appliances,
equipment, materials, and labor required to complete the construction in
a thorough, workmanlike, and satisfactory manner in accordance with the
drawings, specifications, and contract documents, for the following
prices.
