THIS AGREEMENT, made and entered into this 7h day of July , 2004 , by
and between the CITY OF SALINA, KANSAS, Owner, and the above named Contractor, modifies the existing
contract dated May 1", 2004 , under the above subject, project and project number in the
following particulars to wit:
1. The specifications and drawings for certain street improvements in the City of Salina, Kansas, dated
August , 2003 , are hereby amended in the following particulars only; all other terms and conditions remain
unchanged.
ITEM NO. DESCRIPTION
2 Add "Concrete Pavement, 6"" to the north end of the project between Wayne Street and exist
36" end section to improve drainage and future maintenance of ditch, 68 S.Y.
13 Add "Regrade Ditch/Earthwork" beginning at the original north limits of project north 500'
to improve drainage and future maintenance of existing ditch, 75 C.Y.
2. The undersigned proposes to furnish all tools, appliances, equipment, materials, and labor required to complete
the construction in a thorough, workmanlike, and satisfactory manner in accordance with the drawings,
specifications, and contract documents, for the following prices.
