The City of Salina hereby makes application for a permit

to extend its system of sanitary sewers served by a wastewater treatment facility now
operating under Kansas Water Pollution Control Permit No. 0038-474 ,effective July 25
19 86 , in the manner indicated by the plans,specifications, and other data attached hereto.
Any municipality, township, county, or legally constituted sewer district, etc., wishing to
extend its sanitary sewer system is required by`K.S.A. 65-165 and K.S.A. 65-166 to submit
an extension for review by the Division of Environment. Plans and specifications submitted

Approval of Plans and Specifications
for Extension of a Sanitary Sewer
In accordance with the provision of K.S.A. 65-165 and K.S.A. 65-166 as
amended, the Division of Environment has granted approval* to the plans for
MAYFAIR ADDITION and hereby grants permission to City of Salina, City Hall,
Salina, Kansas 67401 to discharge sewage from sewer extensions serving Mayfair
Addition into Salina (Main Plant), and thence treated into Lower Smoky Hill River
in compliance with Kansas Water Pollution Control Permit No. M-SH33-1001,
effective April 1987.
*Subject to the following conditions and requirements: Provide a manhole
on Line MA -1 adjacent to Linden Drive.
We have noted that the project references approved specifications currently
on file with our office entitled: 2AF and Associates Standard Specifications
for Sanitary Sewers KDHE #21169.
We are returning: Two (2) approved cover sheets.
Continuous engineering observation of the construction of the proposed
improvements (observation of building sewer connections shall be the
responsibility of the owner unless otherwise specified in contract documents),
shall be provided in accordance with Kansas Department of Health and Environment
Regulation 28-16-55 and as defined in Water Quality Policy Memorandum #2-78,
effective January 18, 1978.
Notification that the project has been completed in substantial accordance
with the approved plans and specifications must be reported to the Division of
Environment with the attached form.
