Dear Mr. Knox:
The permit you obtained today - Construction Permit No. 12304 -
has been voided. Barry Hummel of our department inspected the
site at 2033 Lewis and advises that the proposed length of
laydown curb and gutter cannot be justified at this location.
A copy of the voided permit is enclosed. If you wish to
proceed with this project, you must obtain a new permit for
the sidewalk and revised approach construction at this
address.

Lot 15, Block 15, Belmont Addition
Dear Mr. Knox:
After our joint field review of your construction work adjacent to
the above referenced Lot 15 several weeks ago, I have concluded
that the curbing placed on Magnolia Road and on Ohio Street does
not have to be removed and reconstructed by you as directed in my
letter dated October 17, 1988. The three (3) deficiencies listed
in the October 17th letter, however, were found to exist.
In lieu of directing you to remove the defective work, we feel
alternative action, a probationary period, is more appropriate in
this case. During this probationary period, with a twelve month
duration, the Engineering Department will concentrate our efforts
to insure that all of the City Codes relating to concrete construc-
tion on public right-of-way are followed. Any variation to these
Codes must be requested by you to this office. Responses to these
requests will be made in writing by either me or Don Hoff.
Any construction work performed by you not in strict accordance
with the Salina Code or without the above written variance approval
must be removed prior to the issuance of subsequent permits by our
office. Failure to remove the defective work within a specified
time frame shall be cause to revoke your bond and concrete license.
In addition to the above conditions, you shall remove and replace
the existing curbing on Magnolia Road between the east end of the
new curbing and the west end of the curb return approximately
14 L. F., located at the northwest corner of Ohio Street and
Magnolia Road to provide a proper transition in alignment of the
north curb line. This work must be completed prior to the
issuance of future permits to you.

It is our sole intent to assist you to perform quality con-
struction work on public right-of-way on all future projects.
We believe the "Specifications for Concrete Work" (Revised
January - 1988) is a clear and concise document outlining
minimum standards for the construction of driveway approaches,
sidewalks; curb and gutter, street repairs, etc. on public
right-of-way. If you have any questions concerning any of the
specifications contained in this document, please contact me.
We, also, encourage you to make an appointment to come into
our office to discuss matters pertaining to this probationary
action.
